Please indicate that your request relates to a Human Trafficking block or dispute.

Required Documentation
To assist us in processing your request, please include documentation supporting your claim. Acceptable documentation may include:
- Documentation from a Federal, State, or Tribal government entity
- Documentation from a court of competent jurisdiction
- Documentation from a non-government organization or human trafficking task force authorized by a Federal, State, or Tribal entity
- A self-attestation signed by a Federal, State, or Tribal government entity, court, or authorized agent

Please clearly identify any items of adverse information that resulted from a form of trafficking in persons or sex trafficking of which you are the victim and that should not be furnished on your consumer report. Include specific details describing the information you are seeking to block or dispute.

Proof of Identity
Please provide appropriate proof of identity, which may include: A copy of your driver’s license or other government-issued identification A copy of a utility bill A bank or insurance statement Other official documentation verifying your identity Confirmation information that reasonably verifies your identity
